HIV - A Geneva patient in remission after a bone marrow transplant

The human immunodeficiency virus (HIV) kills more than 600 people worldwide each year. The case of a patient in remission for 000 months and followed for a marrow transplant at the University Hospitals of Geneva (HUG), in collaboration with the Pasteur Institute in Paris, could help to better understand how to control, or even cure, a HIV infection. To treat leukemia, this patient indeed benefited from a stem cell transplant which, for the first time, does not present a rare genetic mutation that could explain this remission. Which makes it a unique case.

To date, five people worldwide (Berlin, London, Düsseldorf, New York and City of Hope) are presumed to have been cured of HIV infection following a bone marrow transplant. They all received cells with a rare genetic mutation, CCR5 delta 32, which prevents HIV from entering cells of the immune system. In the population, people carrying this mutation are naturally protected from HIV. The virus does indeed use the CCR5 co-receptor to infect immune cells, but the CCR5Δ32 variant of the receptor effectively blocks virus entry. The cells are thus said to be “resistant” to HIV infection. The great scientific interest aroused by the HUG patient lies in the fact that he received stem cells lacking this mutation.


According to Professor Alexandra Calmy, associate physician, head of the HIV Unit at HUG, and Professor Asier Sáez-Cirión, head of the Viral Reservoirs and Immune Control Unit at the Institut Pasteur, “the duration of undetectability after the interruption of treatment – ​​20 months – has no precedent in people who received a marrow transplant in the absence of the CCR5Δ32 mutation. »

The patient has been living with HIV since the early 90s and has always taken antiretroviral treatment. In 2018, to treat a particularly aggressive form of leukemia, he underwent chemotherapy and then a bone marrow transplant to replenish his reserves of blood and immune cells.

From the first month after the transplant, tests showed that the patient's blood cells were entirely derived from the transplanted stem cells. Further testing no longer detected HIV and the patient finally stopped taking antiretroviral treatment in November 2021. Since then, HIV has been undetectable in his body, suggesting long-term remission. The patient is also in remission from his leukemia.

Currently, this treatment protocol cannot be transposed on a large scale due to its aggressiveness. It is considered too risky to treat HIV-positive people who do not need a transplant. The latter can in fact make them vulnerable to other infections, and the body sometimes rejects the transplant. This discovery, however, opens new perspectives for better understanding – and perhaps one day controlling – the way in which the virus replicates in the human body.

For the moment, this discovery only concerns one patient. Additional research will therefore be necessary to determine whether similar results are observed in other people subjected to the same treatment.

This new case of long-term HIV remission will be presented on July 24, in Brisbane, Australia, during the 26th international AIDS conference, IAS 2023, by Professor Alexandra Calmy and Professor Asier Sáez-Cirión.

Kary Bank Mullis, born December 28, 1944 in Lenoir (North Carolina) and died August 7, 2019 in Newport Beach (California), is an American biochemist.

He received the Nobel Prize in Chemistry (co-awarded to Michael Smith) in 1993 for the invention of the polymerase chain reaction.

From 1987, he has been a consultant in nucleic acid chemistry for numerous firms and sits on the scientific council of several companies. In 1988, he analyzed the human immunodeficiency virus (HIV) using PCR. He searches, in vain, the publication affirming that HIV is the cause of AIDS, and alienates a good number of doctors and researchers, "by denying that AIDS is of viral origin".

Another controversy stems from his association with Peter Duesberg, a virologist from the University of Berkeley who denies the viral origin of AIDS. He considers the use of his PCR process in determining viral load, a process for detecting HIV positivity, to be fraudulent. As such, he published an article in the journal Genetica (see bibliography), a journal whose impact factor is 1, which does not classify it among the reference journals.


Both Mullis and Duesberg were considered leaders in their fields and recognized by their peers right down to their position on this pseudo AIDS virus.
The two expressed themselves in videos on how and why they came to doubt the official discourse... like Montagnier also on viruses and healthy living.

And AS BY CHANCE these studies on gains of function focused on coronaviruses.
And AS BY CHANCE no before/after GOF sequencing has ever been delivered.
And AS BY CHANCE in the family of one of the laboratory workers there was a death following the presumed escape from Covid…

Five years after receiving a stem cell transplant, Paul Edmonds, a 68-year-old Californian, no longer has any trace of HIV or the leukemia from which he suffered. He is in remission from both illnesses.

Known as the “City of Hope patient,” his clinical case was described in the New England Journal of Medicine [1].

Stem cells with a rare genetic mutation

After having had HIV since 1988, Paul Edmonds was diagnosed with acute myeloid leukemia (AML) in 2018. He had been taking antiretroviral treatment for HIV since 1997, which reduced his viral load to “undetectable” levels.

In November 2018, Paul Edmonds began reduced intensity chemotherapy. It took her three cycles to achieve remission, which occurred in mid-January 2019.

On February 6, 2019, the patient was able to benefit from a stem cell transplant to replace the cells damaged by chemotherapy. He then received stem cells with a rare genetic mutation, CCR5Δ32. This mutation makes people who carry it resistant to HIV. Indeed, HIV uses the CCR5 receptor, placed on CD4+ immune cells, to penetrate and attack the immune system, but the CCR5 mutation blocks this pathway and prevents the virus from replicating.

Paul Edmonds had few side effects. Since his transplant, he has no longer shown any signs of leukemia or HIV.

The oldest patient

In March 2021, the patient stopped his antiretroviral treatments. During his analyses, no HIV was detected.

The medical team that followed him claims that he is officially cured of cancer, and that it will take another two years for him to be declared cured of HIV.

Paul Edmonds is the fifth patient in the world to be in remission from his acute myeloid leukemia and HIV after having benefited from a stem cell transplant (see Stem cell transplant and HIV: a third patient in remission). He is the oldest to have achieved this, but also the one who has been HIV positive the longest.

“The ‘City of Hope patient’ demonstrates that it is possible to achieve HIV remission even at an older age and after living with HIV for many years,” explains Dr. Jana Dickter, chief clinician at the City of Hope Infectious Disease Department. His case further demonstrates that “remission can be achieved with a lower intensity regimen than the treatment received by the other [previous] four patients.”

New tracks

Dr. Stephen Forman, professor in the Department of Hematology and Hematopoietic Cell Transplantation, says the hospital doesn't plan to stop there. “Our researchers are working (..) to create stem cells carrying the genetic mutation that makes them naturally resistant to HIV,” he continues.

City of Hope researchers have also produced CAR-T cells that can fight HIV-infected cells and “control the virus” in preclinical research (see CAR-T: a clinical trial to treat HIV ). A clinical trial using CAR-T cell therapy, which could allow HIV-positive patients to stop antiretroviral drugs, is expected to begin soon.

In the United States, three HIV patients were also treated using CRISPR-Cas9 (see HIV: a first patient treated with CRISPR). Early studies suggest that CRISPR can efficiently cut sections of the virus's DNA located in human cells. Following the clinical trial, new data on the effectiveness of the treatment are expected this year.

Supplement from 19/07/2024: On July 18, Christian Gaebler, doctor at the Charité hospital in Berlin, announced that one of his patients, a 60-year-old German, no longer had any trace of the HIV virus. in his body after a bone marrow transplant. Diagnosed with HIV in 2009, the sixty-year-old received a bone marrow transplant in 2015 to treat leukemia. In 2018, he stopped his antiretroviral treatment, and almost six years later, researchers found that he no longer had a detectable viral load. This could be the 7th case of cure for HIV.
