Pasteur Lille obtains € 5 million from LVMH to reposition an old drug, Octofene, on the treatment of Covid

The Institut Pasteur de Lille announces that it has received a donation of five million euros from the luxury group LVMH to continue its research on a "molecule particularly effective against the SARS-Cov-2 virus". According to the information collected by AEF info, this molecule is called Clofoctol and was used in France from 1978 to 2005 to treat respiratory infections in the form of the Octofen suppository. A clinical trial on animals and then on humans must validate the relevance of the repositioning of this drug in the early treatment of Covid-19.

The Institut Pasteur de Lille claims to have identified in vitro "a particularly effective molecule" to fight against Covid-19. According to the information collected by AEF info, it would be Clofoctol. This active principle was marketed in France from June 1978 to treat benign respiratory infections. The corresponding drug, Octofen, administered in the form of suppositories, was withdrawn in January 2005 from the French market by Afssaps for lack of efficacy. But it is still used in some European countries. Questioned by AEF, the Institut Pasteur does not wish to "confirm or deny" the name of this molecule.

Can this drug live a second life - and gain worldwide notoriety - by working against SARS-Cov-2? This is the hope of the "task force" which brings together around thirty pharmacologists, virologists, chemists and engineers on the campus of the Pasteur Foundation in Lille (read on AEF info). Among the avenues of research followed by this team is the repositioning of drugs already on the market. A solution which would make it possible to rapidly deploy a treatment since the side effects on humans are already well known.

Tests from a large chemical library

The Institut Pasteur de Lille is working with a spin-off installed on its campus. The company Apteeus has specialized since December 2013 in research on rare diseases: it has created a chemical library of 5 molecules that it tests on primary cells of patients suffering from these genetic diseases ignored by large pharmaceutical companies. When a molecule has an effect, it can be administered quickly. A child has already been treated for several years with an active ingredient identified by Apteeus.

Researcher in an Inserm-University of Lille-Pasteur Lille laboratory, but also founder and strategic advisor of Apteeus, Benoît Déprez suggested to his colleagues to follow the same method to try to find an active molecule against SARS-Cov-2. “We tested more than 2 drugs and at the end of May, a molecule was identified after a cascade of screenings,” he explains to AEF info, Monday, October 000, 12. “This molecule has an action on the two gates of entry of the virus into human cells, unlike hydroxychloroquine. Moreover, it is not necessary to increase its concentration for it to be effective, unlike Remdesivir. "

Decrease viral load

If it proved its effectiveness, this molecule could therefore be easily administered to patients, on an outpatient basis and not in the hospital, just after a positive test. Acting quickly could lower the viral load, reduce contagiousness and prevent the patient from developing a serious form of the disease. To reduce diagnostic time, researchers from Pasteur Lille wish to use saliva tests from the CNRS Sys2Diag laboratory, directed by Franck Molina in Montpellier (read on AEF info).

These avenues of research were presented in May 2020 to Frédérique Vidal, Minister of Higher Education, Research and Innovation, during a visit to the Pasteur Lille Foundation campus. They were also brought to the attention of Prime Minister Jean Castex during a visit to the institute's screening center in early August. “We had discussions but no reactions,” laments Benoît Déprez.

LVMH sponsorship

At the end of September, the foundation made the choice to publicize its discovery, starting with France 3 then by multiplying the interventions on the national media, which spoke very quickly of a possible "miracle molecule". The objective of this intense communication is to find the five million euros necessary to conduct solid clinical trials. The president of the Hauts-de-France region, Xavier Bertrand, reacted on October 1 on twitter by promising financial support. Regional advisers are expected to vote in aid of nearly 800 euros during the plenary session on Tuesday 000 October.

But the main support came from the private sector: LVMH announced Friday, October 9 the payment of a donation of 5 million euros to the foundation. A decision taken by its CEO, Bernard Arnault, after hearing Benoît Déprez in the media. The luxury group, which had manufactured masks and hydroalcoholic gel to fight against the shortage of these products during confinement (read on AEF info), considers "vital that this research can continue". This donation will allow the institute to procure drugs and recruit professionals to support it in clinical trials.

Tests in humans and animals

"Five million euros is the sum necessary to carry out a trial in humans against a placebo and ideally double-blind", explains Benoît Déprez. "It is quite expensive because it is necessary to manufacture a placebo, to draw lots of patients, to have the test approved by the ANSM." In other words, the drug will be compared to a product which looks the same but does not contain the tested molecule, without the patient and the caregiver knowing what is being administered.

The 780 euros from the regional council will be allocated to tests in animals. The experiment should take place in the macaque and be carried out by the Immunity and Transmission laboratory of the CEA, in the Paris region. "Our goal is to spare as much as possible the use of animals by carrying out as many in vitro tests as possible", explains Benoît Déprez. "To estimate the risk / benefit ratio of a new product, it is obvious that it is necessary to test in animals. For a molecule on which we have a hindrance, with thousands of patients already treated, the question may arise. ... But proof of efficacy in animals is a favorable element for pushing a project with the academic community, "he notes.

Do everything to avoid "hysteria"


Why not confirm the name of the drug? "You have seen the hysteria that it can create", replies Benoît Déprez, explicitly referring to the controversies surrounding hydroxychloroquine. "A shortage of the product could prevent us from giving a proper test," he fears. A limited risk, however, since the drug is no longer on the French market.

For the Institut Pasteur de Lille, which is a private research foundation recognized as being of public utility, "the aim is to produce scientifically flawless results, not to engage in a commercial objective", underlines Benoît Déprez. If it is confirmed in the coming months, this discovery would however give a new impetus to an institute which loses each year between 3 and 4 M €.
