Lumberjack, I looked a little at your problem and, in my opinion, the problem comes from the fact that the downloads "by icons" go through javascript unlike the others.
Do you have something (like a "!" Or "X") or "javascript error" panel displayed in the IE status bar (bottom right) when you click on the download icon?
If yes, try to click on it and if a message appears, give us the content, it could help to find the cause of all this.

Yes, I think it comes from java. Often it is an order that is not executed, I encounter this kind of problem regularly.
