Although obviously not recommended, the electrode welding process is remarkably tolerant with regard to the surface finish: it accommodates traces of paint/grease/rust/zinc because the impurities are incorporated into the slag (the surface crust resulting from the melting of the surrounding coating), which is a big advantage when repairing in places where it is not always easy to expose the bare metal properly. In these circumstances, it is wise on the one hand to increase the amperage and on the other hand to ventilate the place, because the fumes of these stains are very toxic...
No point in hoping to use a semi-automatic in such conditions...

The report only confirms what I already thought. But beyond the loss of know-how, I think the main problem is corruption.
Realize a work as complex as a nuclear power plant in a country where shenanigans have become the norm, should not be surprised that it fails...
The blow to the tank and other parts declared compliant but tested non-compliant should have immediately ended the careers of all those involved, with heavy penalties, I do not believe that was the case.
