It wasn't until the US outbreak of 2003 that monkeypox really caught the world's attention. Although the virus is named monkey pox, monkeys are not the source of the virus. Several rodents and small mammals have been attributed as the source of the virus; however, the true origin of monkeypox is unknown. The name monkeypox is due to the viral infection first observed in macaque monkeys. Although human-to-human transmission of monkeypox is very rare, it is usually attributed to respiratory droplets or direct contact with mucocutaneous lesions of an infected individual. At present, there is no treatment allocated to infected people, etc.
Monkeypox is one of many zoonotic viruses belonging to the genus Orthopoxvirus of the family Poxviridae, as shown in Figure 1. Isolated from various animals, Poxviridae viruses are large enveloped double-stranded DNA viruses [2]. The main hosts of poxviruses are rodents, rabbits and non-human primates, which can sometimes be transmitted to humans, facilitating human-to-human transmission [3]. Taxonomically, the Poxviridae family is further classified into two families: Entomopoxvirinae and Chorodopoxvirinae. Classification of the subfamily is based on whether the virus will infect insects, such as Entemopovirinae, or infect vertebrae, as is the case with Chorodopoxvirinae [4]. The family Chorodopoxvirinae is further classified into 18 genera as shown in Figure 1. Each of the 18 genera in the subfamily Chorodopoxvirinae lists several viruses, the majority of which are zoonotic in origin.

The clinical presentation of monkeypox virus is similar to that of smallpox [5]. Monkeypox is less lethal than smallpox with case fatality rates around 10% [6].
Poxviruses are large linear double-stranded DNA viruses with genome sizes ranging from 130 to 360 kbp that replicate in the cytoplasm of vertebrate or invertebrate cells [15-16]...
Due to the larger size of poxviruses, it is more difficult for viruses such as monkeypox to breach host defenses by passing through gap junctions. The larger size of the virus also makes it difficult to replicate rapidly and orthopoxviruses need a more comprehensive strategy to survive within the host [20]. The larger size of orthopoxviruses alerts the immune system of the individual very early and thus generates an immune response very easily [20-21]...
Monkeypox became a disease of global public health importance after2003 due to the first outbreak in the United States linked to infected prairie dogs [9]. Native prairie dogs housed with rodents imported from Ghana in West Africa were thought to be the primary source of the outbreak, as most of those infected fell ill after contact with pet prairie dogs [25 -26]...

Many cases of monkeypox arecurrently concentrated in parts of Europe and the Western Hemisphere [9]. In Europe, current reports show that the highest number of cases are present in the UK. Currently, many confirmed cases of monkeypox are prevalent in people under the age of 40 with a median age of 31 [21]. This is a population born only after the cessation of the smallpox vaccination campaign, which further reflects the absence of cross-protective immunity [20-21]. There is also a higher prevalence of cases of smallpox in males, but the exact explanation for this is unknown.

Since the eradication of smallpox in the 1970s, the smallpox vaccine has also been abandoned. The cross-immunity provided by the smallpox vaccine against other orthopoxviruses began to decline [21]. People under the age of 50 are less likely to have been vaccinated against smallpox and are therefore more likely and likely to be infected with other orthopoxviruses such as monkeypox...

Transmission:

Although human-to-human transmission is less common than animal-to-human transmission, it usually involves respiratory droplets with prolonged face-to-face contact or contact with lesions of an infected person [31].
According to the World Health Organization (WHO), it is not yet known whether monkeypox is sexually transmitted or not...
The most common cause of human-to-human transmission, rare as it is, is through respiratory droplets.
Monkeypox virus follows an infectious pathway similar to smallpox, in beginning with exposure of the host's oropharyngeal or respiratory mucosa...
The clinical manifestation of monkeypox is not visible during the incubation phase and, therefore, the incubation period is not contagious...

Common, non-specific symptoms, as depicted in Figure 6, begin to develop one to two weeks after a person has been infected with monkeypox virus [36]. During the prodromal phase, non-specific symptoms triggering the immune system appear such as fever, lymphadenopathy, myalgia, etc. Due to the nonspecific nature of these initial symptoms, an infected person may attribute these symptoms to the seasonal flu or the common cold.
The hallmark feature of monkeypox is a disseminated vesiculopustular rash [38]....Once the crusted lesions break off and reveal new skin underneath, an individual is no longer considered infectious. ..
The lesions are painful at all stages listed in Figure 7 up to the scaling phase, at which time the scab causes extreme itching in individuals...
