- No scientific basis for a potential therapeutic effect against COVID-19 from preclinical studies;
- No significant evidence of clinical activity or clinical efficacy in patients with COVID-19 disease, and;
- A worrying lack of safety data in the majority of studies.
We do not believe that the available data support the safety and efficacy of ivermectin beyond the doses and populations indicated in the prescribing information approved by the regulatory agency.

Context: It appears that ivermectin has the potential to act against COVID-19 infection. Today, there is an urgent need to assess the efficacy and safety of ivermectin. The effect of ivermectin treatment on patients with mild to severe COVID-19 has been studied.

Method: A 2-day, multicentre, randomized, double-blind, placebo-controlled phase 45 clinical trial was designed in five hospitals. A total number of 180 mild to severe hospitalized patients with confirmed PCR and chest image testing were recruited. Radiographic findings, hospitalization and low duration of O2 saturation, as well as clinical findings such as mortality and blood sample variables were analyzed using standard statistical analyzes in SPSS (V20).

Results:
The average age of the participants was 56 years (45-67) and 50% were women. Primary and secondary results showed significant changes between day zero and day five of admission (∆ 0/5) in terms of ΔALC5 / 0, ΔPLT5 / 0, ΔESR5 / 0, ΔCRP5 / 0, duration of low saturation in O2 and length of hospitalization (CI = 95%). The risk of mortality was also significantly reduced in the study groups.

Conclusion: ivermectin as an adjuvant reduced the death rate, low O2 duration and length of hospital stay in adult patients with COVID 19. Improvements in other clinical parameters have shown that ivermectin , with a wide margin of safety, had a high therapeutic effect on COVID-19.

Trial Registration: This trial was registered on the Iranian Clinical Trials Registry website (Registration ID IRCT20200408046987N1).
